{{Quickfixn}} OnLogout was not triggered

Hi, 
I have experienced a situation that I was not able to receive anything from the server as a client.Apparently I was logged out however the OnLogout method was not triggered at all.
Can someone help to explain a bit about how the heartbeat works in QuickFix/n and what are the possible reasons that the OnLogout was not triggered?
